Key Duties and Responsibilities:
- Face to face Patient case management
- Care Planning & Risk Assessing
- Communicating with MDT’s and other professionals
- Supporting complex care teams to enable our Patients to achieve their aspirations and goals
- Managing Complex healthcare packages
- Mentoring HCA’s to develop clinical skills and ensuring competency
- Training & Developing others
This role involves working from our Norwich office, overseeing care packages and working alongside our existing Children’s nurse. Work is varied and rewarding working alongside families and continuing Healthcare to assess and support carers to implement outstanding care to children with complex health needs. You will have the freedom to organise your own work schedule,

Experience Required:
- Professional Registration with Active NMC PIN
- Experience with Children and young adults (2 years)
- Confidence and Passion to enhance others knowledge
- Experience of training
- Good communication skills
- Tracheostomy and ideally Cough Assist and Ventilation
- Ability to demonstrate leadership
- Drive to achieve better care delivery
