Key Persons in Your Care Journey

We understand that meeting lots of new people at once can be overwhelming—especially for children and families who are already navigating complex situations. That’s why we aim to keep things simple and reassuring from the very beginning.

Your Registered Nurse

Your journey with us starts with a visit from your Registered Nurse, who will carry out an initial assessment. They take the time to understand your child’s medical needs, your family’s routines, and what matters most to you. This is the foundation of the care we provide.

Once funding has been confirmed, your nurse will use the information gathered to create a personalised care plan. This plan is tailored specifically for your child and your family—because no two families are the same.

Your Dedicated Care Consultant

After your care plan is in place, you’ll be introduced to your Care Consultant. This person will become your main point of contact for day-to-day questions, updates, support, and planning. Whether it’s scheduling care visits, arranging holiday cover, or talking through any changes in needs, your consultant is here to make everything as smooth as possible.

They are also your advocate—someone who listens, understands, and responds to your unique circumstances. You can expect regular check-ins, open communication, and a relationship built on trust.

Of course, your nurse is always available for clinical matters, reviews, and health-related support. Together, your nurse and consultant form a close-knit team around your family.

Getting to Know You

Some families enjoy a home visit over a cup of tea, while others prefer a catch-up at a local café or park—whatever feels most comfortable.
